PHOENIX, October 5, 2022 /PRNewswire/ — Valle del Sol, Inc. (“Sol Valley“) a non-profit organization that provides health services to underserved communities in the state of Arizona, has learned of a data security incident that may have impacted protected health information belonging to certain current and former Sol Valley the patients. Sol Valley informed potentially affected people of this incident and provided resources to help them.
On January 25, 2022, Sol Valley discovered unusual activity within his network. In response, Sol Valley immediately took steps to secure its network and minimize the impact of the incident. Sol Valley also engaged a leading independent cybersecurity firm to investigate what happened and determine if any sensitive information may have been affected. Through the survey, Sol Valley learned that protected health information for certain Sol Valley patients may have been accessed or acquired without permission. Sol Valley then undertook a full review of the potentially impacted data to identify those involved, which concluded on July 18, 2022. Valle then immediately began collecting contact information to notify potentially affected people of the incident. The address collection process ended on September 1, 2022. On October 5, 2022, Sol Valley sent notification letters to all potentially affected individuals along with resources to help protect their information.
The following protected health information may have been involved in the incident: names, dates of birth, social security numbers, driver’s license numbers, clinical or diagnostic information, health insurance member identification number, medical record number and Medicare or Medicaid number. Sol Valley has no evidence that this information has been misused.
Sol Valley takes the security of its patient information very seriously and has taken steps to prevent a similar event from happening in the future.
