To record your blood pressure or take an ECG with your new Galaxy Watch 4, you need to download the Samsung Health Monitor app. The Samsung Health Monitor app runs on both the phone and the watch and syncs data between the two, but before you can use it, you need to meet a few requirements.

For starters, you can only use Samsung Health Monitor with an unmodified Samsung phone. Other brand device or rooted Samsung device will not be able to run it normally. There is also an age limit, which only allows you to take action if you are at least 22 years old. These restrictions are probably related to regulation. Fortunately, it is possible to bypass these restrictions so that you can activate ECG and blood pressure readings on your Galaxy Watch 4, regardless of the device it is paired with, your age, or your region.

A workaround of these restrictions has already been discovered for the Galaxy Watch Active 2 and Galaxy Watch 3 via a modified version of the Samsung Health Monitor app for phone and watch. Characteristics and limitations

Since this is an unofficial modification of the Samsung Health Monitor app, not everything is working properly. Although root checks, country restrictions, age limits, and update prompts have been removed, some functionality is currently discontinued.

At this time, you cannot sync Samsung Health data between your phone and Galaxy Watch 4. You will need to copy this data manually. It is also possible that the application crashes during the first launch or after installation.
